Most people assume Medicare will be simple… until they actually have to make decisions.
The truth is, there are a lot of little-known rules that can cost you money if you don’t understand them.

1. Medicare Is NOT Free
Many people assume Medicare is free—but that’s not true.
- Part A is usually free
- Part B has a monthly premium ($202.90 in 2026)
- Supplements and drug plans also cost extra
👉 You need to plan for these costs.
2. You Can Be Penalized for Signing Up Late
If you don’t enroll on time:
- Part B penalty = lifelong increase
- Part D penalty = lifelong increase
👉 These penalties never go away and increase over your lifetime.
3. Medicare Doesn’t Cover Everything
Medicare does NOT cover:
- Dental
- Vision
- Hearing aids
- Long-term care
👉 Many people are surprised by this. (Some medicare advantage plans do offer Dental, Vision, Hearing)
4. You May NOT Need Medicare at 65
If you’re still working and have employer coverage:
- You may be able to delay Medicare
- BUT only if your employer plan qualifies
👉 This is where a lot of people make mistakes.
5. You Can Change Plans Every Year
You’re not locked in forever.
👉 Every year (Oct 15 – Dec 7), you can:
- Switch Advantage plans
- Change drug plans
You cannot switch Medicare Supplement plans unless you pass the medical guidelines or use the Birthday Rule.
6. Medicare Supplements Don’t Include Drug Coverage
Medicare Supplement (Medigap) plans:
- Cover medical gaps
- Do NOT include prescriptions
👉 You need a separate Part D plan.
7. Your Income Can Affect Your Premiums
Higher income = higher Part B and Part D premiums.
👉 This is called IRMAA (Income-Related Monthly Adjustment Amount)
8. Not All Plans Cover the Same Doctors
With Medicare Advantage:
👉 Networks matter (HMO vs PPO)
If your doctor isn’t in-network, you could pay more. If you do go with an advantage plan, make sure to know the network!
